New Patient Registration
The doctors welcome new patients who live within our practice area. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire. These are available from reception or you can download a copy here. A separate form is required for each family member.
To ensure eligibility for NHS services you will also need to provide photographic identification (passport/driving licence etc.) and a utility or other official document confirming your address.
If you are registering with a doctor for the first time after leaving the Armed Forces you will need to provide the relevant supporting paperwork such as F.med 133, certificate of discharge and any medical records.
Medical treatment is available from the date of registration. Please contact reception for further information.
