Forms and Medical Assessments
Our practice offers a range of non-NHS administrative services. This work takes place outside our NHS time, and therefore fees are often applicable.
All forms are completed at the discretion of the GP, and if we are unable to complete your requested form we will contact you.
Specialist Assessments
Our GPs are trained to provide general medical services and support within the NHS framework. Specialist assessments should be undertaken by professionals with specific expertise in those fields.
Therefore, we do not undertake private work that may involve specialist assessments or activities which present a significant risk of vicarious liability. These include, but are not limited to, the following:
- Fitness to fly assessments
- Clearance for high-risk sports (e.g., mountaineering, skydiving, paragliding)
- Certification for participation in sporting events (e.g., marathons, triathlons)
- Diving medicals
- Medical certifications for extreme sports or adventure travel
- Driving assessments beyond completing DVLA requests or those listed below
- Court of Protection Forms, Mental Capacity Act Assessment for Solicitor’s forms (COP1/COP2)
To have these forms completed, patients are advised to seek independent medical assessments from specialists in the relevant field. These might include:
- Occupational health specialists
- Sports physicians
- Aviation or diving medical examiners
- Travel medicine clinics
- Solicitors
Blue Badge and PIP requests
Applications for Blue Badges are dealt with by the local council: https://www.hants.gov.uk/transport/parking/bluebadge/howtoapply
For Blue Badge and PIP forms they often ask for a patient summary which we can provide free of charge. Usually if they require any further information they will contact us directly. If you feel you need a GP letter as supporting evidence there would be a charge for this.
Specific Medicals:
We do provide medicals for:
- HGV (D4)
- Taxi drivers (HFDC)
- Entrance to Armed forces
- Adoption medicals (AH1/AH2)
- Fostering medicals
For D4 and ML5 forms you will need to have a vision assessment at an optician before seeing the GP for a medical.
Travel Cancellation
Please check with your insurance company if they need a letter from your GP or if they will need a form to be completed which is more common.
The form will need to be submitted to the surgery so your GP can complete it and there is a fee.

GPs can no longer act as passport signatories unless they know the applicant in a personal capacity:
